Medium, large and giant breeds are all geriatric at 12 under the AAHA guidelines, because they have lived past their average lifespans of 12, 11 and 8.5 years. Toy and small breeds, averaging 14 and 13 years, are seniors at 12 and will become geriatric at 14 and 13.

According to the AAHA life stage guidelines, a dog enters the geriatric stage when it outlives the average for its size. At 12 in dog years that means:

  • Giant (94): geriatric for three and a half years, an exceptional age for a Great Dane or Mastiff.
  • Large (84): geriatric for a year.
  • Medium (74): geriatric as of this birthday.
  • Small (64): senior, geriatric next year.

Care for a geriatric dog is about the day-to-day: warm bedding, rugs on slick floors, a raised bowl, help on stairs and a consistent schedule. Why 12 in dog years isn't 84 for every dog

The seven rule gives 84, which matches the large breed figure at 12 but is 20 years too high for a small dog and 10 too low for a giant. The UCSD epigenetic clock gives about 71 for a 12-year-old dog, illustrating how the molecular and life stage models drift apart in old age.

The AVMA calls the seven rule a myth because it ignores both the front loaded maturation of the first two years and the effect of body size. By 12 a Chihuahua and a Saint Bernard are 30 human years apart on the size-adjusted chart. Kraus and colleagues (2013) found that large dogs age faster, with each 4.4 lb of mass costing about a month of expected lifespan, which is why so few giant breeds see this birthday.

The epigenetic estimate of 71 comes from Wang et al. (2020), whose formula 16 ln(age) + 31 was fitted to methylation data from Labradors. Because the curve is logarithmic it barely moves after age 10, so it understates the life stage of a 12-year-old large or giant dog.
